Want to play fall high school sports? Turn in your forms and pay the fee

In order to begin practice for fall sports on Aug. 11, all Chaska High School athletes in grades 7-12 must have three forms completed and turned in to the activities office by the morning of Aug. 4.

The Chaska Activities fall form and fee, the Minnesota State High School League ethics code must be read and signed by the athlete and a parent, and an up-to-date physical must be on file with the high school activities office.

“Otherwise, it will be no practice/no play for athletes missing any of these forms or the activities fee,” Athletic/Activities Secretary Connie Palmer said. “More importantly there could be health and safety concerns for the athletes if they participated without these completed and on file.”

District 112 requires a physical every two years. A physician may use their own form or the one on the MSHSL form.

All forms can found in the hallway just out side of the CHS Activity Office, near the West entrance.

The Activities Office is open weekdays from 7:30 a.m. to 3 p.m., and can be reached at (952) 556-7130.

Fall activities are: football, boys and girls soccer, volleyball, girls tennis, girls swimming and diving, cross country and cheerleading.
